no metronidazole for pups in Gastritis

Dear Swarup, you are right. All the cases of GE do not need Metronidazole. but when you are sure that the stools are foul smelling, watery to semisolid, it is advisable for gut acting antimicrobial especially to check growth of anaerobic microbes.

A 13 yr old intact male boxer had wound on the neck.it did not heal with regular dressing and antibiotic therapy i.e ceftriaxone @20mg/kg bd for 10 day actually no improvement. then another wound appeared over the shoulder. how to proceed?

for chronic wound ,use oxoferin drops (2 drops ) topically and regular dressing without using anythg else.for another wound check for drainage tract(fistula) b/w two wounds and clean using same procedure as aove.

this is a case of recuurent abcess, continue ur antibiotics for many more days. dress twice a day. allow drainage. use something that will not allow the pet to self mutilate itself. use amoxyclav tid for 7 days with probiotics. you will get better result.

hi i think you should debried the wound n then go for the normal dressing n you should try oint placentrex for dressing. n change antibiotic to augmentin 375 mg bid with some enzymatic preparation like chymoral forte twice a day. best of you . lage raho

stop all medications,clean the wound with saline and hydrogen peroxide for a few days, after a gap of 5 days take a culture and then debride the wound and start medications. cultures are very important aspects of todays treatment esp with drug resistant bacteria on the prowl.

Some informations on MCT:
1avg age for mastcells: 8-9yrs
2 mast cell tumors are found in or under the skin on the trunk of the body itself, and the vast majority of the remainder are found on the extremities, especially the hind limbs. They are less commonly found on the head and neck, and less commonly still arise from tissues other than the skin. If they are very swollen or ulcerated, there may be pain, but most MCTs are unlikely to be painful

vaginitis with HD

TDX: Hip dysplasia+ vaginitis. Tx: Megaflex, Betadine pressary alt days+syp citralka. Owner not willing to take megaflex telling its on Puppylarge breed from Eukunuba!+ pettabs.Adv X-ray to rule out Hip displasia.

yes . it could be both the things i. e hip dysplasia and vaginitis. before starting megaflex we can take one x ray of the hip jt. at the same time we can start meloxicam for 5 day s . if it is confirmed as HD then we can start megaflex/enhancer /maxima along with care o pet(carprofen). if the problem is too severe then even the supplements will not help. we have to go for surgery i.e FEMORAL HEAD OSTECTOMY.owner was not willing to take megaflex bec u did nt confirm HD.X RAY IS MUST IN THIS CASE.

Renal failure in a dog with low TLC

DrKumar Ravi hi docs i have a case of great dane who hao acute renal failour with b/t reports of last week as tlc-4800, n-20 l-80 blood platlets-1.20 lack serum creat-4 bun 120 vomitting off feed. after treatment with iv fluids, conciplex, rantac, lasix, enrocin to combat sec bac infection as its a viral origin of renal failour. n...ow he start taking food little bit no vomitting. today i repeat the blood test s.creat rise to 5.1 tlc 4200 l-75 n-20 blood plat- 80000. what should i do im totally confused . i recheck the samples but results r same

dr i dont know whether its a case of acute renal failure of viral origin or not. acute renal failure is a rare condition. and viral origin is still rare. if it is acute then the onset will be sudden,with olig uria or anuria mostly. reason could be poisoning, nephrotoxic drug, leptospirosis etc.whether there was polydipsia/polyurea, what was the hb ... See Morelevel? what was the temperature?urinalysis would have been a good option. u can see the specific gravity of urine, casts,pus cells etc . it can be prerenal azotemia also.
as per the reports which we have ie platelet count 1.2 lakh and then 80000. leptospirosis can be the cause.
treatment options will be fortified procaine penicillin, lasix,rantac,emeset/ondansitron(if there is no loose motion)and enrocin along with aggressive fluid therapyi.e normal saline @ 25 ml/kg bw bd. if there is no appetite then 5%
dextrose, and amino acid solution (astymin)should also be given. blood test after 3 days again to see the parameters i.e creatinine and bun. if it does not come down then prognosis is very poor.then peritoneal dialysis can be tried.

There is drug called AZODYL ( gut acting pre+probiotics which reduce Ammonia production in gut ) on TID orally can reduce Sr Creatn level drastically. stop enrofloxacin as it is excreted through kidney, use omeprazole alt days with sucralfate for saving GI tract. Use phosphate binders tab phostat675mg bid po. use herbal liver tonics for appetite.
... See Moredo not stop fluid. use RL 500ml 2 bottles + D25 2 bottles. check for sr protein ,if below 5 go for iv Aminoacid supplements, its not contraindicated. some people uses envas tabs to see that there is bettre perfusion in kidney. can use this too and its very safe.
Make a chart of all drugs, tell clearly how and when to give which medications. need more details ?

one more thing, this may be a case of just renal failure post meloxicam pain therapy!! low TLC is not always indicative of viral renal failure.am I wrong?

yes, dewormig will be a good answer. another problem is whether we shd give fluid therapy in gastroenteritis when there is pneumonia? and what will be the best antibiotic -cftriaxone and gentamicin, or amoxy clav or something else?

If it is a pleral effusion, I wont give much fluid but if lasix is added I will use fluid as per requiremnts. In preumonias one can easily use fluids.it helps only.
gentamicin if given dehyration should prefrably not there.

This was a very interesting case, of Pyrexia of unknown origin. The TLC was 22,000, the platelet count 2 lakhs. the fever never came below 106F, and i had kept it on ceftriaxone 500 mg( 32 kg 1 yr gsd).
occult blood was positive

chest x ray as above can be seem to have multiple patches and severe congestion.
the patient was put on cefaperazone sulbacin for 7 days and has shown complete clinical recovery.

I had seen a case like this which had a fever of 106+ , it was a siberian husky. vomiting, bleeding stool, and temp never fell down. We put him on p[aracetamol once in 6hrs, dicrystin , enro & oxytetracycline. dog died. Diagnosed as ICH or collangitis!
most dogs find very difficult to survive at 106 + temp if it continues for more than 2 hrs and if you use NSAIDS you find bleeding !!

The dog died under vet supervison, tests tells that!! Must be he had respiratory problems due to low HB and all problems are related with it. you wrote ESR as 75. Can it be possible?

as per the report which says tlc was 49400 i personally feel 3 possibilities i.e leptospirosis, tick fever or internal abscess. other parameters rule out 1st two possibilities.there was a very high probability of abscess in lungs

doc,even i thought its severe leucocytosis so some pus filled infection,and after draining the back abscess and pus thought to taken hold of root cause ..laboured breathing was intermittent.i thought may be becoz of pain in legs and back region.so ,i agree there might be high poss.of abscess in lungs.then we had to take care of low hb and severe infection .what more cud have been done here??

as far as treatment options are there only symptomatic treatment was possible. but we can help the owner if we give a good opinion abt the prognosis of the case. an x ray of the chest would have been a good option.
