interesting case,please keep updating doc.
use some other antibiotic also. why not try DEC? avl in tablets for phylariasis.
I think Lymphnode enlargements are mostly due to localizeed reasons.

Padma Yadlapalli
Sorry for my ignorance but what is DEC
was the edema localized to only that limb;
Also he seems so anemic with that RBC coount any other pathology noted on the smear; were you able to get a chemistry or profile done; what was his albumin or TP if so

@padma yes the edems was localised on that limb.
@swarupmay doc, the draining tracts on the edematous leg disappeared after itraconazole treatment, still continuing with it, the disappearence of draining tracts , i thik is supporting pythiosis as the culprit

viju, why did u choose itraconazole over fluconazole or ketoconazole, itra is very costly for long duration treatment which any fungal ailment normally is?? please tell me why u preferred itra? did you choose it because of the more convinient dosage regime of 5mg/kg/day?/

any history of colitis, pythiosis is also associated with LI signs? U should have done a culture as systemic mycosis cases are not so reported in INdia and it would have been reslly helpful. What could have been the source of the infection? Zoonoses?

not sure abt the reason. pythiosis occurs in 2 forms the GI form as well as the cutaneous form
. i think it might be the cutaneous form, as, so far the animal is responding to the Tx

the animal responded well to the tx.
the edema subsided. but unfortunately died yesterday, respiratory trouble. i thik it would have been the consequence of the systemic form that might have been present. any comments or suggestions

not sure. the owner didnt turn up. might be organ failure, but it had respiratory trouble and i thought it might be because of the disseminated nature of the disease, i mean systemic affections..

thats the possibolity no 1, but it was an eye opener case

SC Pythosis form is really rare and respiratory form is really rare too :)
ReplyDeleteso here you are talking about two rare things that occur at once, not making since for me.
Purulent ocular discharge, this is abnormal, popliteal LN enlargement is not normal, responding to Itraconazole give rise to 3 things:
1) Fungal, which I cant see the ulceration on the limb, also respiratory form is rare and cutaneous form is rare and it is almost impossible for 2 rare conditions to occur at the same time.also it doesn't explain the other clinical signs.
2) Cancer some cancers respond to Itraconazole as it has anti-angiogenisis effect but i think the effect occur so soon.
3) The Edema disappeared by chance while other clinical signs were developing.
4)It was pythosis and another thing but you used ABiotic I think.
Still a huge Question any Idea's
Why ocular discharge occur, Anemia? and DEATH!!!!!
Thank you a lot for the posting of comments
